Added patch for CGI front-end to report fatal errors instead of staying silent and leaving it to the web server.

Brought up to date for version 2.11+2.111-1.

Finally, FromCGI seems functional and the documentation is accurate at
first read.  Added "add_to_from_cgi" because it was too hard for Drinker
to drink a Pint without it.  And it was easy.  Still need official
tests. Arghh.

Drinker uses add_to_from_cgi and other stuff. Now it is a nice working
basic example with lots to do such as berrs, brewery, adding handpumps

Fixed FromCGI and AsForm some more. No official tests in crud.t yet but
i have a working demo and will move code to synopsis or tests.

The macros, i just tweaked it a little so it would display and array in
view_item  and make a default name out of column name if colnames has no
entry for "column".
