Subject: [PATCH] Use with statement instead of temporary table

SQLAlchemy might use server-side cursors and prepend a

  DECLARE "..." CURSOR WITHOUT HOLD FOR

to the statement.  This does not work if the statement starts with creating a
temporary table.
